It doesn't automatically mean anything negative will happen. It does however give ISPs the power to charge you for different "packages".
In theory they could make you pay a separate fee if you want to access social media, or nerf your speeds if you don't pay. Then a different packages to access spotify/pandora. Then a different package to access porn. Etc.
You could very quickly rack up a huge monthly bill.
